Check your grip at impact fix. It sounds like the hands are working too hard for clubface control - they are clamps - and the proper alignments at impact fix are the only sure way to allow them to act like clamps!

I actually feel the activation of the thenar muscles during impact fix. This gives me more precise alignments at address.
Non adjusted address such a lagging clubhead takeaway is when my hands feels more like clamps and the thenar muscles tighten up during the takeaway.
